RN-Multiple Perm Positions

on Healthcare is currently looking for RN's for multiple perm positions located in all 5 boroughs of NYC. Responsibilities: Responsible for complete care management of members needs and coordinates services with providers. Collaborates with Medical Doctors in delivering care to patients. Responsible for adequately addressing the progression of the patient's illness and the corresponding home care services required. Coordinates provisions of all ancillary services as well as health care services not included within the HomeFirst benefit package. Travel to patient homes is required. Required to participate in educational programs and professional committees. managed care nurse, managed care rn, nursing, rn, nurse, care management Qualifications: Associates/Diploma from an accredited school of Nursing; BSN preferred. Basic knowledge of Microsoft Word, Excel and e-mail skills. RN licensed to practice in NYS. Demonstrate effective critical thinking. Ability to effectively communicate with all members of the healthcare team. Demonstrates ability to problem solve.
