Manager of Care Coordination $110K-$125K

escription *Accountable for the day to day direct management and oversight of Clinical Care Coordinators and for performing duties inherent in a managerial role *Organizes and conducts orientation of new Clinical Care Coordinators and Program Assistants. *Conducts ongoing training and mentoring. * Assists Director, Network Integration and management team with introduction and implementation of cost containment and conservation measures. * Assists with network Integration with design and implementation of data collection processes, insuring adequate and accurate data. Makes suggestions for data collection which enhance care coordination activities. *Develops methods for evaluating staff performance and measuring and monitoring department care coordination objectives *Share plans and ideas with Director to insure that employees across the department receive consistently clear information, direction, and assistance. *Makes skilled patient assessment / reassessment including medication management, psychosocial needs, self-care, skilled or home nursing services and follow-up appointments. *Assists the Director, Network Integration in managing resources to meet budgetary goals while responding effectively to necessary program changes and altered staffing levels. *Schedules and utilizes staff with flexibility so that the workloads are equitably distributed and contractual obligations and productivity goals are met. Qualifications *Licensure as Registered Professional Nurse in New York with current registration * Bachelor's in Nursing. Master's in Nursing, Social Work, Public Administration, Business Administration or related field *Minimum of three to five years of clinical nursing experience *Five years or more of supervisory, management *Minimum of three to five years’ experience in care management/coordination for a health plan or hospital. *Must understand health insurance including Medicare and managed care requirements and Affordable Care Act initiatives
